A rare autoimmune condition explained the patient's dry mouth and decay around previously well-maintained PFM crowns in the posterior. Tetracycline staining had always kept her from smiling fer fullest, but look at her now!

Case was done digitally under conscious moderate sedation in phases, #29 was non-restorable and is healing for future placement of an implant.

A win for same day dentistry and pontic site development!

Patient presented with cc: "my front tooth is dropping!" #8 showed mid-root horizontal fracture with Class II mobility and was deemed non-restorable. The thought of losing a front tooth is scary, but same-day dentistry (in this case 3D printing), saves the day! EXT, bone graft, PRP of #8 combined with a provisional pontic nested into the extraction site let the patient leave "like it never even happened." 

Once the pontic site was healed and developed, the final zirconia bridge hides the secret that one tooth is indeed a "fake!" Keeping the soft tissue contour helps us keep the secret--papillae are preserved! 

The patient is so happy with their updated smile, their lip is travelling higher! With pontic site development, we were able to help the zenith of #8 more closely match #9.

Crisis averted!

Sneaky recurrent decay nearly ruined this patient's family reunion! Patient presented as a limited exam with #10 broken off at the gumline and a cross-country flight tomorrow morning. We were able to extract #10 and 3D print a temporary bridge #9-11 (both had recurrent decay under old restorations too). Now the talk of the reunion will be adventures of the last year--and not her teeth!

Peg laterals gone by Prom

18 y.o. patient with peg laterals wanted a more permanent solution to discolored bonding for prom--this weekend. 

CADCAM made same-day transformation possible, eMax characterized to match areas of mild hypocalcification for an awesome result!

A better match!

This patient became discouraged with the cervical darkening of #9 (recession + recurrent decay + previous RCT), and wanted a better match between the centrals. Same-day Zirconia provided opacity and a life-like match with contours and an incisal edge that mirrored #8.

Careful extraction!

Stresses the importance for a bone graft after extraction even if an implant isn't planned! Preservation of the buccal plate upon extraction and graft of #7, #8 and immediate provisionalization with pontic site development led to life-like pontics and a better soft tissue outcome.

Same day!

This patient moved from out of state with what she described as "temporaries", patient was discouraged by the staining and thought they looked "boxy." Same day zirconia gave the patient an esthetic outcome in just a few hours!

No more gap!

This patient always disliked the gap between their front teeth, but didn't want to pursue ortho or extensive crown and bridge. A waxup was designed for a "trial smile" to see if widening the centrals would give an esthetic result. The patient was so pleased with the trial, we proceeded to finals!

Game changer!

#8 was no longer salvageable, was extracted and grafted in preparation for an eventual implant. In the meantime the area is protected and shaped by a temporary bridge #7-#9.

In four months, an implant will be placed, and the patient will continue to use the temp bridge until each tooth is ready for an individual restoration.

This patient's sister was getting married in a few days and wanted to smile in photos, but hated the gap betwen his front teeth. With selective reduction and widening the laterals, the diastema could be closed without a "buck teeth" look--check out these same day temporaries!
